Annual Individual Personal Planning Week APRIL 14 - 22
~Statewide Calendar Events~
Governor Susana Martinez has designated April 14-22, 2012, as Annual Individual Personal Planning Week - (see Proclamation). This coincides with National Healthcare Decisions Day, April 16th, and this week in New Mexico includes Advance Health Care Directives, as well as powers of attorney for other matters.
We encourage everyone to sponsor events in their communities to discuss the importance of communicating and documenting their arrangements in advance for their future healthcare decisions, etc. We also encourage everyone to use the tools provided below and on our "Alternatives to Guardianship" video page.
Also a sample of a personal laminated wallet-size card regarding Health Care Power of Attorney for emergencies, and a sample template for a two sided 3x5 information card that can be put on the refrigerator. The 3x5 card is for Para-medics to get information quickly. These can be in a magnetized plastic holder.
This can be a significant community service for our friends and families. We need community leaders and others to organize community-wide campaigns state-wide for our Annual Individual Personal Planning Week - April 14-22, 2012.
